Change effective-date from dashboard UI #258

Allows user to change the end-date of the 1 year reporting period from any provider dashboard.

Home controller seemed like the most appropriate place for setting the effective date. That is only for UI purposes; one can call for queries through the query controller specifying the effective date already.
