This basically takes all the definition lists and turns the items into subsusub(...) sections. I think it works much better for a few reasons:
The right sidebar works really well as a way of navigating the spec now
We can easily refer back to the definition of a thing (say, start_time) in the MDM from other parts of the document.
This is a big win I think and should make it fairly easy to starting filling in the remaining gaps.
There's probably a follow up in which we go through things, linking back much more agressively than we're doing now. I just did it for a few things to illustrate and see what people think.
