Add birth surname #173

Hi! Thank you for this wonderful CV!

I would be grateful if someone could suggest how could I add birth surname to a CV header. So far I have figured out this \name{Name}{Surname \\ \Large{(f.d. BirthSurname)}} But I don't really like how it looks.
