SYNC - Add master flag to resources #965

The master flag will be true when the resource is created on this server instance, and false when it was created from a sync operation from another server. The resources will be master on that server.

account (contains organizations, user, role, api keys, etc.)
ehr
contribution (contains versions)
query
ehr query
opt index
folder

With account we can sync organizations and users, so there is no need to set master on those.
