Settings options suggestion #166

Please add a settings option to toggle:

"Display all footnotes and cross-references for the whole of the current 
chapter."

Default should be FALSE (as present behavior - just for the current verse).

NB. When TRUE, each note in the list would require an origin reference, not 
just the note tag.
