Change the fixed width of the column based on the SITE_CONTACT_DISPLAY field

Old display: urns display Intermediate fix: urns_display_2 New fix: contact_display_fix

This is to fix the display of urns contacts and ensure that the labels are aligned. @projectmushroom please review. @rowanseymour please provide your feedback as well. This should address your response on https://github.com/rapidpro/casepro/pull/186
