Note: This is another PR after the project's migration to react based project. The previous PR #61 had merge conflicts because I forgot to pull the changes.
Also Note: The screenshots and video provided were actually for the previous PR, but after the project has migrated it has got a lot of issues in the old directory files like images are not showing up, and CSS is not getting applied because of the changes in the file structure, those issues can be dealt in another issue.

This commit will improve the UI of
the Bed Availability Notification Form page
and also make it responsive.

I have changed the HTML code of the page and divided it into various divs to apply my CSS
I have changed the CSS file of the page, added media queries, and removed some unnecessary code from it.
