No formatting or concealing of italics and so on in blockquotes #583

I am not sure that this is a vim-markdown issue, and I don't know how to check whether it is, but my italics appear in italics with the underscores concealed, expect inside block quotes.

Same happens to me when trying to make a word bold inside a sentence in italics words. Something like: _This is a **sentence**_ It's shown as: This is a **sentence** Here in github it works fine: test word
