Record where new word found

Record the page or request where each new word was found.

(Possibly in the dictionary with the word or maybe by having
each word in the "+show trace"  header being a link to
the provenance of that word.)

Would this be for debugging purpose?  If so, does it work for you if you can 
extract this information from the debug output ?

Ping?  If for debug purpose: I can add a print statement.  If for a different 
purpose then I'm not convinced this is worth the effort.

Ping timeout.  Feel free to reopen in case you'd be happy with this being in 
the debug output.  If it is for general scans, feel free to provide a patch. I 
think it's a lot of effort which is probably better spend on adding new 
injection tests.
