Formatting and presentation improvements #29

This contains a number of presentation adjustments:

re-order the left hand menu to raise importance of websites and tools
combine the "contact" page and "how to use website" into a single "about" page
add a last published date in a footer
hide the table of contents on pages that don't populate it
added a heading to gateway page so it appears in table of contents
