Integrate RTT capabilities into the debugger extension.

This branch also includes minor changes to reflect updates to the probe-rs-debugger functionality.

Release of corresponding functionality in probe-rs #688
Update of the probe-rs webpage with the new instructions for VS Code extension
Publishing the probe-rs-debugger-0.2.2.vsix to the Releases page of this repo
Need an updated GIF to show capabilities in README file
