Description of Change

Create a Sign-up screen for Admin and Non-admin users. The following fields need to be added and will need to authenticate with the Auth Provider of our choice.

Signup link should take the user to a new form to enter information and sign up with us.

Fields:

User Name (Email Address)
Password
Sign in link (If you already have an account)
First Name
LastName
OrganizationName
Phone - optional
Sign-up Button
