79 problem generating cda document for ems #85

There were various problems when generating the CDA document "Arztmeldung" for EMS. A big problem was that the element "recordTarget" was not present due to the expansion of the elements "{urn:hl7-org:sdtc}:deceasedInd" and "{urn:hl7-org:sdtc}:deceasedTime".
