Set some text fields in surveys as required fields #445

Waiting on list of fields. Drop-down fields will not be set as required fields.

Add new required field at end of all surveys stating:

Have you provided all the information available for these colonies and are ready to submit the survey? Surveys cannot be edited after submission.

This is blocked pending confirmation.

Confirmed. The only required field should be a new field added to all surveys asking:

Have you provided all the information available for these colonies and are ready to submit the survey? Surveys cannot be edited after submission.
