Update data fields #507

Make two field changes to monthly survey:

  1. Under "where did the queen come from?" add an option for "Local, non-commercial or reared on site"

  2. Change "Do you treat for Varroa" to "Did you manage for Varroa since the last inspection? Select all that apply."

Log this time as support hours.
