Add QI-Core Required Values to Patient Resource on FHIR Bundle Export

During Patient Export as FHIR Bundle:

• Add a Medical Record Identifier value equal to the internal patient ID to the Patient Resource.
• Add QI-Core patient profile url to the Patient Resource.
