Show button to create payment status pages when at least one page is not set or does not exist anymore #103

Fix #101.

One remaining 'issue': the parent page for the payment status pages — which does not have a stored page id option to check for — is also recreated if the action to create pages is performed a second time. Thoughts?

Removed the top level page.
