Hello,

before @jaugustin isCrossRef patch the form widgets were created for many to many tables with more than 2 columns too.

Even after the BC patch to restore the old m2m behavior without isCrossRef it ignores m2m tables with more than 2 columns.

This pull request fixes this issue.
