orders_v1_6.fla - New services

I believe inside orders_v1_6.fla you'll find tabs for each service Protec offers. e.g.:

• ALS - Address Location Search
• CHC - Criminal History Check
• SSN - Social Security No. Verification
• MVR - Motor Vehicle Record
• etc.

(1) Protec wants to add a new service tab to this application:

• PRR - Professional Reference Review

(2) And wants these modifications to existing tabs:

• SOS - Add a new state: “AJ - All jurisdictions”
• ARS - Abuse Registry Search: When the state Nebraska is chosen, no longer offer separate Adult and Child checkboxes. There should only be one checkbox labelled "Adult + Child", and it should be checked by default.

(3) And wants new "states": CN - Canada and PR - Puerto Rico added for the following services and costs:

• Canada: CHC $60, ECR $25, AEV $15, PEC $15.
• Puerto Rico: CHC $25.
