Deny approval #722

Add Decline option for approvers so that they can Approve or Decline.
Once declined it should be flagged as such in the shopping cart.

Tested on /mageimage/
Order_approval  0.2.0.0alpha2 (alpha)

Added 2 items in my cart. Declined them both as the Approver. Nothing changes 
in the customers cart.
Shouldn't the items be removed from the cart? The just stay there and the 
message says "Please, wait for your purchase to be approved." when I click 
"Proceed to checkout"

Also, on the approval page, the action (Approve / Decline) happens as soon as I 
pick one from the drop-down menu. No buttons or anything. Looked confusing, was 
expecting to hit a button after I chose what to do with the items. Dunno, what 
do you guys think?

Yeah, we need to discuss messages in different cases.

I don't think item should be removed as soon as it's marked as declined. 
Customer can edit declined item or remove it and start from scratch.

That's how it's done in magento.
Look, there're mass action and per item action, same as in product's list in 
admin interface.

1. Items that have been declined show up in the shopping cart marked as 
"Declined" (declined.png).
If I go through with the ordering process, the last message I get just right 
after I place my order is that "Unapproved items remain in my cart". But the 
shopping cart is empty.
What about leaving the unapproved items in the cart with a "Edit" link next to 
the "Declined" flag. So that users can edit the order and file it for approving 
once more.

2. In the approver interface, there's a drop down with the two actions for 
individual approvals (approver.png). It used to action the choice as soon as it 
was selected. Now nothing happens and there's no button.
What about we add a Approve / Decline buttons in the place of the drop-downs?
