What if you booted with PADEMU disabled?
OPL Stable 1.1.0 SMB - works well
OPL Beta 1.2.0 2017 SMB - works well
My PS2 model is SCPH-90008. I use redump images and don't use PADEMU, VMC, GSM.

Silent Hill 2 Greatest Hits [SLUS_202.28]:
OPL Stable 1.1.0 SMB - works well

I tried disabling them all and same error. It only boots 1/18 times after repeated tries with vmc pademu gsm off. Both SMB and MX4SIO
Guess ill be more specific and say I have a PAL SCPH-90004
Can you try with Opl beta 1968 version? Same thing happened to me on USB. With the 1968 version the game worked without problems. Then I switched to one of the newer Opl updates and it didn't work anymore which is why I'm playing the PAL Director's Cut version which works flawlessly.

PAL Director's cut works with latest too, we want the Greatest Hits NTSC (The best version) to run which is bugging me out.

I would try the beta you mentioned but I sadly cannot find it.

The reason I'm playing Silent Hill 2 Director's Cut now is just that, Greatest Hits gave me black screen in the last updates. I don't remember exactly from which version, if I remember correctly I went from 1968 directly to 2004 and it no longer worked for me, the exact same backup of the game I was playing with the 1968. I have been playing Director's Cut ever since and have never played Greatest Hits again in recent updates. In any case, I play from USB and you from SMB, maybe things could be different.

@KronaDComics, on the net they write that this game is incompatible with some PS2 models and some modchips. Disable or remove the modchip if you have one. Also check this version of the game, it is also NTSC and has English language.

Checked with modchip disabled entirely, same thing happened where it boots 1/4 attempts. Yeah i'm gonna try a different version.
