Remove Windows 2012 R2 Support

Remove support for Windows 2012 R2. The last vagrant box (from us) was updated over 2 years ago and the newest PT versions are not supporting it.
Bump the box version to use the latest Windows 2016 box (1.0.4) with Jan 2020 patches.
