Tribe selection should be optional

What steps will reproduce the problem?
1. Patient registration screen No.1
2. Without selecting one of the tribes from tribe list data entry not
possible now.
3. Also the 2nd screen of the registration "Patient Information" is
overlapping the "Billing" tab. 

What is the expected output? What do you see instead?
There may be some general patient also. So it should not be mandatory.

What version of the product are you using? On what operating system?

Please provide any additional information below.

Try revision 232.  Should've fixed both the billing button and the tribe save 
error.
