Update OTM name #806

Above the form for the invention disclosure number we have: 

 Authors using this option are required to file an Invention Disclosure form with the Intellectual Property Office in order to obtain an Invention Disclosure Number

The name "Intellectual Property Office" is no longer used. It is now "Office of Technology Management". We should make this change for any instances of "Intellectual Property Office".

Also, they plan to change the name again to “Office of Technology Transfer”, but they don't know when that will be.
