Update algorithm from latest change in spec #91

- Removed the second note in the definition of sx:sync/@id in section 2.4 
which stated that two entries in the same feed with the same atom:id should 
be considered different sync items.

- Added a new step 3.2.4 to the update algorithm. Without this new step, an 
endpoint could briefly get into an inconsistent state. This new step 
eliminates that possibility.

Second point seems to be particularly important.
