(fix): Update broken links in index.html page

Fixes #568 - Also put all changes from PRs https://github.com/publiclab/community-toolbox/pull/572, https://github.com/publiclab/community-toolbox/pull/573, https://github.com/publiclab/community-toolbox/pull/574 together into one PR.
