Refactor html pages #108

Resolves https://github.com/publiclab/simple-data-grapher/issues/107

I've moved all of library scripts and styles into the <head>, leaving the rest in the body. There was not title element in the original code, so I wasn't sure what to set as the title.

@debck could you take a look at this please? Thanks!
