Below is text that will lend some meat/orientation/humanity to the great structure that we have built so far.

Checklist of below items:

Above first question in each section of the questionnaire:

Welcome to the x section of the questionnaire!

Answering these questions can be a bit boring at times but it’s very important that you take the time to carefully read every question and answer to the best of your ability. If you don’t know the answer: no problem, just click the “I don’t know” option. Only skip questions if you object to answering the question. (also see symptom section specific prompt in the google form)

@NovakN is that the right way to treat the skip question?

Progress indicator:

Reads: Approximately 18 questions left.

Should read: Approximately 18 questions left in this section

If you leave the questionnaire and then return:

Thanks for circling back to the questionnaire!

Please take the time to carefully read every question and answer to the best of your ability. If you don’t know the answer: no problem, just click the “I don’t know” option. Only skip questions if you object to answering the question.

Above middle question

You’re half way there! Keep up the great work!

Forum 1: Share your symptoms story

Congrats on finishing the symptom section of the questionnaire! It is by far the longest section and it’s an important one. Standardized surveys are important for quantifying your experiences but they don’t do a great job at representing the full depth of your experience.

Please take a minute or two to write down you health related experiences that you think are relevant. Be sure to note important things that you think we left out of the questionnaire.

While the symptoms questionnaire was just about one person. These stories can be about your whole family or the experiences of people who just come to visit.

Below the text input field

After you share your symptoms story you can read and comment on the stories of other Where We Breath members.

In the symptom forum

You and X other people have helped the public to better understand this issue. Explore their stories to find someone with similar experiences.

You can comment on any story (including your own).

Data (same size font as forum)

Down here you’ll find the real-time results from the questionnaire. They are presented here in graph form to protect anonymity. Check back in a couple weeks to see if the patterns in the data change. If you notice any interesting trend or connections in the data below or the stories above share your observation in another story »

below the graph

You can use the “next question” or “previous question” links down here (arrows) to move between graphs.

Don’t forget that there are more sections of the questionnaire and all of the pieces of the puzzle are important!

following questionnaire text should be the same minus “It is by far the longest section and it’s an important one.” At the start of the non-symptoms forum.
