Current state of graph design from #3:

I think the short survey list looks concise enough to move forward on -- thank you so much, Nicole.

I'm counting ~9 questions, if some are paired, and if the big grids are not broken up. Would we show users how far along they are?

Is there a listing of all models? I think a nested dropdown where we show models for each make is a good idea; let's get that list together.

Year of mfctr - we may want to use a dropdown to reduce entry error. Range? 1950-2014?

For "Which of the following describes this housing unit?" - would this be redundant if someone enters the make/model? Or could we auto-fill it if they do that completely?

Is City | State | Zip Code redundant and could we just do zip code?

Here is the only model list I can find. Its HUGE. And also does not include anything that would have VIN number (Like travel trailers) http://www.chattelmortgage.net/make_model_reference.html

There is also this list of manufactures: http://www.mobilehome.net/manufacturer.htm And we could go to all of their websites and pull model names...

For a lot of the older units, I think people won't be able to find the model and also our contemporary lists might not have older models on it. Also we will want to contrast symptom and formaldehyde levels across the general type which relates to "Which of the following describes this housing unit?" So I think that info is worth keeping for now. For me having more questions on the survey would be much more important than all the time necessary to figure out what class of home each of the models is. But then again maybe it would be pretty straight forward as we are likely to find listings of the different models all withing discrete categories (like the travel trailer list will be on a travel trailer site).

Ok. Just got off the phone with a contact of mine who sells RVs in Mississippi. It looks like getting an exhaustive list is going to be pretty hard. He thinks http://www.rvia.org/ might have a list (not on their website). but probably unlikely. He says that Winnebago is the only company that has good data. And every model since 1999 you can enter the vin number in and get the model et cetera. But that's the only company with their data straight.

I wonder if we should just focus on getting the manufacturer down as that in itself will be a big task. We should be able to figure out the model of the units from the VIN number.... although that might not work for the mobile homes/manufactured housing that use HUD numbers. Just thinking about ways to save resources.

Was thinking that "Next question" is not very obvious. Perhaps we can have a bar above the graph saying "You've answered 5 of 12 questions. Move to the next question >>" or something.

That sounds great. Also maybe it could just automatically move to the next question after 15 seconds--with of course an option to return to the graph?

This now needs a mockup of the question form for new question types, and graph display of a different kind of data. Nick, can you classify the existing questions into different categories so we can try to standardize their display a bit? Yes/no, multiple choice, choose from a 1-5 scale, that sort of thing? would most work in a bar graph display?

Sections: (internal names, we can make them more accessible/sleeker down the road) 1) About the house and household (to be filled out only once per user) 2) Individual Health Effects (repeated) 3) mitigation (repeated) 4) Other Indicators of Poor IAQ (repeated) 5) Demographics and Other Risk Factors (once?)

Will they display well in a graph form: not all of them. Some it might be easiest just to have a number, like "5 of 213 other WWB users are also living in this make and concerned about their air quality" (eventually we can have a "connect with them" feature).

For the ones that are hard to graph because of too many possible answers, we could also only graph the top five responses in relation to the individual so that all of them could be graphs?

My totally subjective take on graph vs number and the form of each question

Q 1: Make--drop down menu--number Q 2: Model--multiple choice with "other" field--number (we are unlikely to be able to secure all possible models, so we can maybe put some of the most popular models in a multiple choice with an 'other' field, but we are likely to get some user error in entry and thus a difficult graph.) Q 3: Year of manufacture--drop down menu--graph Q 4: Category--multiple choice with 'other' field-- graph Q 5 : Unit zip code location--five digit number--oof this one is tricky. Maybe we should have state here as the legal boundaries of the state might be more important than the exact proximity of the zip code and zip will already be asked in the log in, no? Q 6: Use--multiple choice with other field--graph Q 7: Brought from who--open-- number (very unlikely to have any overlapping entries here unless doing field work or once we have large numbers) Q 8: Fema--multiple choice-- graph Q 9: Number of inhabitants--number Going to just post to save

More thoughts after having gone through the survey a couple times.

1)In terms of mitigation time question I think we should have one more column of 3 days to 6 days between one day and one week as I think most people I talked to found it was two to three days that people started feeling better. 2) we should include companion species early in phase II as they are really important sentinels 3) How long has this person lived in the unit? is in both the symptoms section and the final section---is that intentional nicole???

Wow, this is looking great.

Re: Nick's questions: 1) That would be no problem to add an additional response, I was just guessing (The mitigation time question does not come from a standardized survey--that one is ours). 2) I will start looking into the pet symptoms literature 3) The question in the final section is referring to the person's paid occupation, not their residency--I was modeling those questions off of this survey: http://www.stopmcs.org/?page_id=1165. I can change the wording so it is clearer.

Additionally, I realized people will probably want to have data on race/ethnicity. Do you guys mind if I add that in, and does Public Lab have any opinions/guidelines/standard methods for talking about race/ethnicity? If not I will go ahead and do what we usually do in epidemiology.

Jeff, should I make these changes in the google survey we already have up? Would you prefer I make a copy and work off of a new survey draft?
