Subject headings should only look at LCSH and name headings should only look at NAF #9

The examples below are related to Issue #18 "Deities, fictitious names, and legendary characters from LCSH to NAF." Fictitious names are part of NAF, but they used to be in LCSH. Some bib records have retained the cancelled LCSH control numbers in $0.
