Update README, add CHANGELOG #73

This is now ready for review.

[x] Update README to clarify installation
[x] Prettify README (using code sections, titles, etc.)
[x] Add CHANGELOG
[x] rename all indentation settings to be prefixed by g:
[x] create a vim manual from README (generated via script)
[x] test indentation still works 😅
