Allow spaces in image filenames #143

What steps will reproduce the problem?
1. Create an ePub with spaces in image filenames
2. Upload ePub
3. Navigate to a page containing that image with a space in the src (%20)

What is the expected output? What do you see instead?

I expect to see the DocBook Stamp image. I see only the alt text.

The problem was actually that BW wasn't unencoding the @href in opf:item, and 
so it
was looking for a file literally named 'the%20stamp.png'.  Fixed in rev 730.
