Refactor books such that only a single copy of each unique work exists #99

Right now each uploaded ePub is a discrete item.  There are like 500
identical copies of that Pride & Prejudice edition.

There should only be one representation for each unique book.  If the user
is able to edit metadata (in the future) then the book should "fork."

This needs to be solved in concert with the dc:identifier problem. [#98]
