Release multi-instance support #96

This PR is releasing the following PRs:

92 Use Instance ID given in the notification for reporting purposes
93 Make Internal Device State Store scoped per Instance
94 Make token provider scoped per instance
95 Enforce singleton to use one instanceId

Plus updating the changelog.
