Add unscanned books and data

This requires an ORM replacement for the current metadata scan OR adding the folders for each input book. The latter is interesting because it would put them in place ready for adding the scanned content but we are probably going to have to go to a relational table at some point.

I sent a csv file with just the metadata fields, is there a possibility that can be implimented with the fields. I checked the add book feature but I don't see all the fields there. I understand the import file didn't have all the fields but I would like to have all the fields for the records that we will be adding manually.
