'Media Change' Notifications Fix #323

Please replace useless header telling us 'Media Change' with name of
song/video. Its taking too much time for searching the name of new
media/song/video under header, written with small letters. Thank you.

I went ahead and changed this, I'm concerned what streaming media will look 
like however.
