Create a new issue template for generic issues #21

Create a new issue template for generic issues including prompts in comments for summary, done criteria, and whether or not the requester wants to work on it.

@Marie-L If someone is new to Github and the project, I wonder if they'll know what the labels should be. Do you think a default label for new, untagged issues would help? Like "new", "untriaged", or something similar? Or are you okay with some issues not having labels if people are not familiar with our labelling system?

@albendz Yes you're right and yeah I can create the label "new", but if it's not used or the issue is left untagged then that's fine
