Initializing CoC in governance repo #1

This PR includes:

governance/conduct/ subdirectory
Within the subdirectory the following documents:
- Code of Conduct
- Code of Conduct reporting

This structure is based on that of the jupyter project.

@sjsrey With this PR, I will go ahead and close out all other PRs and Issues related to the Code of Conduct in pysal, pysal.site, and pysal-githubio. Do this seem OK?
