Create github templates #12

This adds templates for PRs as well as three types of issue. They are borrowed from the https://github.com/tvkitchen/tv-kitchen project

Gone are the day of having to remember what kind of information to include in a code process action!

Again: since this is just a single dev project I'm going to merge this directly for now (@kfogel at some point we may want to talk about options to get at least one other set of eyes on PRs)
