[Guidelines] update substitutions and include new icons #661

Stated in pull request for #658 some icons are missing in Substitution Guidelines. I will add new icons and substitutions so they can be used to update manuals.

Finished the analysis and see that for the Identy results, I need to replace a few icons. I searched for them in QGIS repository, but was not able to find these. Put a request for assistence on Gitter and will wait untill I receive a response.
