DOCS: adds default "community health" files #5

This adds default community health pages and a org-level README.

I'm not certain that I have the links between these documents correct, and I'm not sure how we could test that prior to merging, so it may be that we need to test just after merging and fix if necessary.

The current situation where I'm the only one who can access the code-of-conduct email address is really not ideal, but I don't have a better solution right now - I'm very open to suggestions on that.
