signup error #226

What stepsu will reproduce the problem?
1. goto Signup page > fill in only the first form.
https://test.tradiechoice.com/index.php/signup

Either leave second optional form empty, or just select avatar picture.

2) But always press the second Submit button, at the bottom.

3) Registration does not go thru, it just goes back to the same Signup page, 
preserving all fields values.

What is the expected output? What do you see instead?
