Chameleon Schemas aren't preserved during conversion to XSD #155

Both RelaxNG and XML Schema support chameleon schemas (included schemas that 
don't declare a namespace so that they automatically take on the namespace of 
the schema that included them), but during the conversion process to XSD the 
schemas end up being explicitly rather than implicitly namespaced.

I'm happy to try and fix this bug myself, but really just want to know whether 
this is an intentional decision or not. Perhaps there are certain design 
constraints that have made this design choice necessary?
