HandleInvalidParameter and HandlePureVirtualCall do not initialize EXCEPTION_RECORD properly. #392

What steps will reproduce the problem?
1. Simulate invalid parameter error.
2. Setup FilterCallback to filter by address.
3. Run program.

What is the expected output?
FilterCallback able to detect address of error.

What do you see instead?
EXCEPTION_RECORD::ExceptionAddress always NULL.

What version of the product are you using? On what operating system?
