add modal to set commit title & message on save #442

currently you have to use the auto-generated messages, sad! let's make it so users can set their own commit title & messages. This modal should have a clear message that says "you can leave these blank! Qri will generate a commit title for you", and a link to some as-of-yet-unwritten doc on good commit messages
