Overhauled how deaths and respawns work. This fixes a lot of bugs when dying in a DLC location.

Fixed a bug where players would sometimes be kicked after the statue cutscene.

Fixed errors after the statue cutscene.
Removed startup popup for QSB changelog, because it was too annoying to keep updated.
