Minor improvements to the gmp.hpp file

Replaced some successions of functions by combined functions
Remove repetitions in the code by precomputing additional constants (shift_modulus_shoup)
Consistency (i -> cm)
Careful computations of mph_init2()
Use of mpz_inits() and mpz_clears()
