Remove bug submit form (mod_reportproblems)

Need to remove support ticket form that is loaded on every page via the mod_reportproblems module. We are getting WAY too much spam because the form is on every page.

To-Do:

• [x] Pull in core changes to com_support into app/components/com_support
• [x] Turn off/remove mod_reportproblems
• [x] Remove module load from template
• [x] Redesign support ticket submission for users not logged in (put inputs from User Information on same line) (template override)
• [x] Redesign /support page (template override)
