Please align all the form elements in a consistent fashion following the two column layout

Changes in authorQwizbook page

  1. change audio, link, image and video to buttons similar to Questions.
  2. Give separation beween each references.
  3. Give delete/remove option for dynamically added fields ( like audio, video, link, image in Questions and References) and delete remove option for each references.
