Bugfix/issue 80 #81

Addresses the main problem reported, as well as two other problems identified by the build and check process (also, a couple of misc edits from last summer!).

Installed size is 6.8 Mb due to 5.1 Mb in docs, but the tar.gz is 4.1 Mb. Don't remember if/how we work around this. Build and check does generate a note for this.

Some checks will succeed when #83 is merged.

I'll merge this into https://github.com/r-hyperspec/hyperSpec/pull/83 hoping that some failures or warnings will get solved.
